Mafia: The Old Country is a linear Sicilian crime story of about 12½ hours (HowLongToBeat, via Deku Deals), and its knife exists to solve a problem its guns create: you run out of bullets. 2K’s own Q&A calls the knife “a great option when an enemy gets too close or if you’re trying to preserve ammo,” and that is the rule this guide builds on. Start on Medium unless you know the genre, open Free Ride from the main menu after Chapter 2 (it is free), and treat Man of Honor ($9.99 on Steam) as a roughly three-hour detour that slots between Chapters 5 and 6. Only 7.5% of Steam owners hold its completion achievement, against 60.7% who finished the story.
Below you get a first-hour checklist, a difficulty table with Steam completion rates, a decision table for gun versus knife, how Free Ride unlocks its weapons, and where Man of Honor sits in the story. This is a decision guide, not a chapter walkthrough: it does not cover collectible locations or every boss. Facts were checked against 2K’s patch notes through the 1 September 2026 update, and Steam figures were read on 19 October 2026. For frame rates and graphics options, see our Mafia: The Old Country best PC settings guide.

Quick Start: Your First Hour in Seven Steps
Seven settings and habits carry most new players through the first chapters: set difficulty, make the interface forgiving, learn Instinct, equip a Charm, restock your blade, open Free Ride at Chapter 2 and save Man of Honor for after Chapter 5. Do them in this order.
- Pick a difficulty before Chapter 1. You can change it from the pause menu later (patch notes describe switching between Medium and Hard mid-run), so a wrong guess is cheap. The table below shows what each setting changes.
- Soften the inputs. Turn on Simplified Struggle in the accessibility settings if rapid button prompts bother you (Pixel Twelve). Driving has Manual Shifting and Simulation Driving toggles (2K quick tips), and the November 2025 update added a first-person driving camera.
- Use Instinct before every room. It lets Enzo see through objects and detect nearby enemies, which tells you whether to sneak or shoot.
- Equip a Charm. 2K calls Charms “especially valuable long-term purchases” because they give passive bonuses, and extra slots are sold at Pasquale’s store.
- Pick up whetstones. They restore blade durability, which you spend on stealth kills and throws (details below).
- Finish Chapter 2, then check the main menu. Free Ride appears there once Chapter 2 is complete.
- Hold Man of Honor until after Chapter 5. Its main menu entry needs Chapter 5 completed, and in a New Game it plays between Chapters 5 and 6.
Mafia: The Old Country at a Glance
The campaign is short by modern standards and the price has already dropped: Steam lists the base game at $49.99 and shows $29.99 (40% off) on 19 October 2026. The table gives the figures a buyer needs.
| Fact | Figure | Source |
|---|---|---|
| Story length | 12½ h main story, 14½ h with extras, 24½ h completionist (PCGamesN’s review run: 13.5 h) | HowLongToBeat and PCGamesN |
| Structure | 15 missions: a prologue and 14 chapters | Space4Games chapter list |
| Critic scores | Metacritic 73, OpenCritic 74 | Deku Deals |
| Steam reviews | 22,635 of 30,989 positive (73%, Mostly Positive) | Steam |
| Steam Deck | Unsupported (graphics performance) | Steam |
| PC minimum | Ryzen 7 2700X or i7-9700K, RTX 2070 or RX 5700 XT, 16 GB RAM, 55 GB SSD, 1080p Medium | Steam |
Which Difficulty Should You Pick?
Pick Medium if you have played a cover shooter before, Easy if you have not, and leave Hard and Classic for a second run. Difficulty here changes damage and accuracy, not the story: on Hard, 2K says your weapons deal less damage and enemies hit harder and with higher accuracy, and on Easy your weapons deal more damage. Classic, added in the free November 2025 update (2K’s announcement), goes further: enemies are deadlier, health regeneration is limited, there are fewer HUD indicators, some aim-assist options are off, and unspent bullets are lost when you reload.
| Setting | What changes | Pick it if |
|---|---|---|
| Easy | Your weapons deal more damage | You are new to third-person shooters or want the story first |
| Medium | The middle setting; 2K does not publish its numbers | You know cover shooting and want the intended middle |
| Hard | Weapons deal less damage; enemies hit harder, more accurately | You will use the knife and cover seriously (the “True Soldato” achievement) |
| Classic | Sparse health regeneration, bullets lost on reload, fewer HUD indicators, reduced aim assist | Second run, and you want ammo management to be the game |
Steam’s numbers show how few people take the harder routes. The achievement statistics put “finished the story on any difficulty” at 60.7% of owners, “finished on Hard” at 11.2% and “finished on Classic” at 2.7%. Because a Hard or Classic finish also counts as “any difficulty,” the ratios hold: about one finisher in five did Hard, and about one in twenty-two did Classic. Steam does not publish its denominator, so read these as relative sizes, not exact player counts.
Guns Run Dry, So Learn the Knife: When to Switch
The knife is not a fallback for every weapon; it is the answer to two situations, an enemy at arm’s length and an empty magazine. That is by design. Popverse’s PAX East 2025 report says bullets are scarce, quotes Hangar 13 president Nick Baynes on the game being set before the Tommy Gun existed, and has him saying “the knife is more than a one-shot kill,” with some knives thrown and others made for undercover work. On Classic the squeeze tightens, because reloading with rounds left in the magazine throws them away.
Most players never build the skill. On Steam, 76.1% of owners hold “Cold-Blooded” (30 headshots), but only 26.1% have 20 kills with knife takedowns, and 3.6% have performed every special action five times with each knife. That reading is mine, not Steam’s: the gun is the default and the blade is the minority skill, which makes it the cheapest upgrade to your play.
| Knife | Best use | Trade-off |
|---|---|---|
| Scannaturi | Thrown for a ranged stealth kill | Lower durability; you must recover the knife (2K, Pixel Twelve) |
| Rasolu | One-hit kills in active combat | Each one-hit kill uses durability |
| Stiletto | Long fights; highest durability | Chosen for staying power rather than a special trick |
Durability is the hidden cost. One guide lists lockpicking, quick stealth kills, Scannaturi throws and Rasolu one-hit kills as the actions that consume it, with whetstones restoring it, and says a depleted blade still works for plain melee but more slowly and less quietly. That breakdown comes from a single guide, so treat it as likely rather than confirmed. Enzo carries one knife at a time, so choose before a fight, not during it.

Here is a rule that follows from those sources. It is our synthesis, not 2K’s:
| Situation | Reach for | Why |
|---|---|---|
| Enemy unaware, a place to hide the body | Stealth takedown with a knife | Saves bullets and avoids the noise that pulls in more guards; costs a little durability |
| Enemy alert at range, ammo in the gun | The gun, from cover | A thrown or melee approach exposes you for no gain |
| Enemy has closed to melee range | Rasolu one-hit kill, or the Stiletto if the fight drags on | This is 2K’s “gets too close” case |
| Magazine empty, no pickup nearby | Knife, then loot | This is 2K’s “preserve ammo” case |
| Blade durability gone | Plain melee, then a whetstone | The special actions are what drain it |
Knife duels: dodge the red glint, parry the rest
Duels are one-on-one boss knife fights, which Baynes called “new to the game and new to Mafia.” The first big one is L’Ombra at the end of Chapter 2 (“Palio”), per DTGreviews. Phase 1 is parry-centred; if he guards a lot, a heavy strike breaks it. Around half health, phase 2 adds red-indicator attacks that cannot be parried and must be dodged sideways or backward, then punished in his recovery. Gamer Blurb and 2K’s tips agree on the rule: a red glint means dodge, anything else you can parry, and mashing buttons fails.
One cross-reference note: 2K says button-mashing will not win a duel, while the L’Ombra guide describes quick-time prompts near 50% and 0% health where mashing restores Enzo’s health. These do not conflict if you mash only during the prompt and play the rest of the fight with timing.
Stealth, Driving and Dinari in Brief
Stealth rewards patience: stash bodies where guards will not find them, throw a coin or bottle to pull a guard out of position, and look for a second way in, since some sequences have several entry and exit points. A few story sections require staying undetected (Pixel Twelve), so a loud approach is not always available. While driving, the road signs react to your position relative to your next objective. Dinari, the in-game currency, buys weapons, cars, horses and Charms, and 2K’s advice is to spend it, since unlocked items are saved to your profile and, per the September 2026 patch notes, previously unlocked outfits, hats and haircuts are available when you start a New Game.
Free Ride: Free, Opens at Chapter 2, and Locks Its Best Items Behind Challenges
Free Ride is the open-world sandbox, and it is a free update from 20 November 2025 that replaced the old Explore mode. It has no bearing on the story (2K’s notes say so directly). It opens from the main menu after Chapter 2, and its challenges unlock by story progress: you can hop between the campaign and Free Ride from the pause menu, or finish the story first and start with every opening challenge available. Each challenge pays out at three reward tiers in Dinari, and the new items appear in Pasquale’s store in sequence as you clear them.
| Challenge type | Count | Who gives it | What it tests |
|---|---|---|---|
| Circuit Race | 3 | Pasquale | Laps against rival drivers |
| Road Sprint | 3 | Pasquale | Point-to-point races |
| Time Trial | 6 | Pasquale | Checkpoint and freeform routes against the clock |
| Horse Race | 3 | Gaspare | Lap races using spurs |
| Standoff | 5 | Valerio | Timed waves; looted stopwatches add seconds |
| Assassination | 4 | Valerio | Stealth: execute a target, crack safes for score, escape |
That adds up to 24 base-game challenges by our count of 2K’s list. What they unlock matters more than the count: 16 outfits plus 3 hats, 4 knives, 3 guns, 3 Charms and 2 cars, all bought with Dinari after specific challenges. If you want a new knife or gun for your loadout, you have to open Free Ride to get it.
I cannot tell you how many players miss these unlocks, because no source measures it. Steam does show how rarely the shop’s late stock is reached: only 6.0% hold “The Finer Things” (bought every car from Pasquale). Steam cannot say whether that is because players skip Free Ride, so I am not claiming it. What I can say is that a first run through the 12½-hour line does not require any of it.
My suggestion, which is opinion and not 2K’s advice: skip Free Ride on a first run if you want the story unbroken. If you are struggling with ammo, a Standoff is a good place to practise, because it forces the weapon and positioning management that the campaign teaches slowly. Free Ride also adds a Sundial for weather (Sunrise, Afternoon, Foggy, Ashfall), Photo Mode and the black-and-white Cinema Siciliano mode, all usable in the campaign.
What 62 Achievements Say About How People Play
I pulled Steam’s global achievement percentages on 19 October 2026, and three things stand out. Players shoot far more than they stab. Very few people take the hardest difficulties. And the paid DLC is the least-finished large piece of content.

Even the first achievement, escaping the mine in the prologue, sits at 94.7% rather than 100%, which tells you the denominator includes owners who barely started. That is why all of these are better read as relative shares than as completion rates for committed players.
Man of Honor: Where It Fits in the Story, and Whether to Buy It
Play Man of Honor after Chapter 5 and think of it as a three-hour side trip, not a longer campaign. It launched on 14 August 2026 for $9.99 on Steam (Tech Times rounds it to $10) and adds two story chapters. 2K’s notes give two ways in: pick the Man of Honor entry on the main menu once you have completed Chapter 5, or start a New Game with the chapters enabled, where they sit between the original Chapters 5 and 6. (The AU Review says you need to have “reached” Chapter 5; 2K’s “completed” is the safer reading.)
In story terms, the chapters are set in the winter of 1905 (VGC), after Enzo has been inducted as a soldato and before the 1906 counterfeiting plot. They team him with a younger Ennio Salieri, who Wikipedia identifies as the Don of the first Mafia game, recently out of prison and asking Don Torrisi for help with old scores. The two chapters are listed in walkthroughs as “Intermezzo I: L’Immacolata” and “Intermezzo II: Fratellanza” (2K’s own notes do not print the titles), and they are built around a stealth hunt, a farm confrontation, a mine ambush and a club robbery. You can jump in cold from the menu, and Impulse Gamer describes them as an Intermezzo in the middle of the main story.
The base-game mechanics do not change. Gaming Trend gives it 85/100 and calls it “more of the same, and that’s perfectly okay with me” at about three hours; The AU Review gives it 4 out of 5 and says 2 to 3 hours. The real addition is in Free Ride: Salieri’s warehouse in Porto Almaro offers jobs (cracking safes, stealing high-value cars, hauling stolen goods) and challenges tuned for higher difficulty, plus Wanted Posters, Stunt Jumps and Sulphur Trucks to destroy (2K’s gameplay details). The new gear is 3 guns, 3 knives, 5 Charms, 3 vehicles, 2 horses, 3 outfits and 5 hats, and one of the knives, the Rimbalzu, bounces to a second target when thrown (PlayStation Blog).
Should you buy it? Critics like it, but Steam players are split: 362 of 621 reviews (58%) are positive, which Steam labels Mixed, against 73% for the base game. The completion achievement is held by 7.5% of owners. A caveat on that gap: the base game’s 60.7% finish rate and the DLC’s 7.5% share one denominator, so roughly one story-finisher in eight has finished Man of Honor, though not every DLC owner has finished the story first. At today’s Steam prices the base game plus Man of Honor costs $39.98 (our sum). 2K lists a Definitive Edition that includes Man of Honor and replaced the Deluxe Edition, but I found no separate Steam listing or price for it, so check the store before assuming a bundle saving.
| Buy Man of Honor if | Skip it if |
|---|---|
| You finished the story and want three more hours of the same stealth and shooting | You have not reached Chapter 5 yet |
| You like Salieri or want a link to the first Mafia game | You want new mechanics; reviewers say there are none |
| You want the extra Free Ride jobs, challenges and weapons | You found Free Ride repetitive; Gaming Trend says the open-world activities can get “a tad redundant” |
Which Advice Fits Which Player?
Everyone benefits from the knife rule, but the second hour differs by player type.
| Player type | Start here | Leave for later | Why |
|---|---|---|---|
| New to cover shooters | Easy or Medium, Instinct before every room, one Charm | Hard, Classic, Free Ride | Damage and accuracy settings are the biggest lever, and only 11.2% finish on Hard |
| Mafia series fan | Medium or Hard, then Man of Honor after Chapter 5 | Classic until a second run | The DLC is the one piece that connects to the first game through Salieri |
| Completionist | Story, then Free Ride challenges in order for the shop unlocks | Nothing; do the DLC last | 24½ hours is the completionist estimate, and only 6.0% bought every car |
| Short on time | The 12½-hour main story, nothing else | Free Ride and Man of Honor | Neither affects the main story |
FAQ
How long is Mafia: The Old Country?
About 12½ hours for the main story and 24½ for everything on HowLongToBeat, with PCGamesN’s review run at 13.5 hours. The spread comes from difficulty and how much of Free Ride you do, so plan for 13 hours on a first run.
Do I need to play the other Mafia games first?
No. It is a prequel set decades before the first game, so it stands alone. Returning names such as Luca Trapani (father of a character in the first game) and Leo Galante are extras for fans, and Man of Honor leans on Salieri, who matters more to players of the first game.
Is Mafia: The Old Country open world?
No, the campaign is linear with some side paths. The open world is Free Ride, which has no story and exists for challenges, collectibles and exploring the Valle Dorata, so call it a sandbox next to a linear game.
Does Mafia: The Old Country run on Steam Deck?
Steam rates it Unsupported because of graphics performance, and the PC minimum is an RTX 2070-class GPU with 16 GB of RAM. I would not count on a good handheld experience, and our PC settings guide covers what to turn down on weaker hardware.
Is Free Ride a separate purchase?
No. It was a free update for all owners on 20 November 2025. Only the Man of Honor additions to Free Ride, such as Salieri’s warehouse jobs, require the paid DLC.
Where to Go Next
If the open-world crime fantasy is what you are after, Mafia’s sandbox is not it, and our GTA 6 hub on what is confirmed versus rumor covers the one that is. For a shooter built around loot instead of scarce ammo, see our Borderlands 4 complete guide. Mafia launched in August 2025, so it is not in our best games of Q1 2026 roundup, but that list is a good place to look once you finish. For a chapter-by-chapter route and collectible locations, we have no spoke yet; this hub is where future Mafia: The Old Country guides will link back.

I've been playing video games for over 20 years, spanning everything from early PC titles to modern open-world games. I started Switchblade Gaming to publish the kind of accurate, well-researched guides I always wanted to find — built on primary sources, tested in-game, and kept up to date after patches. I currently focus on Minecraft and Pokémon GO.
