Millicent’s questline is one of the few quest chains in Elden Ring where the point of no return doesn’t announce itself. You can walk into the Drainage Channel in Elphael, see two summon signs sitting a few feet apart, and lock in an ending before realizing the other one is gone for that character slot. Part of the problem is mechanical: Millicent’s position only updates when the game reloads the area, so a step that looks skipped is usually just a step you haven’t reloaded past yet.   1. Kill Commander O’Neil in the Swamp of Aeonia (Caelid) for the Unalloyed Gold Needle.
  2. Give the needle to Gowry at his shack, then rest at a Site of Grace and return — he hands back the repaired needle.[5]
  3. Take the needle to Millicent at the Church of the Plague, east of Gowry’s Shack, and use it on her.[1][5]
  4. Rest again; she relocates to Gowry’s Shack. Talk to her, then rest once more — Gowry becomes a spell merchant, confirming the questline is live.[5]
  5. Get the Valkyrie’s Prosthesis from an upstairs chest in The Shaded Castle (Altus Plateau), guarded by Cleanrot Knights, and hand it to Millicent at Erdtree-Gazing Hill.[1][6]
  6. Follow her through Windmill Heights and Ancient Snow Valley Ruins, exhausting dialogue at each Site of Grace.[1][6]

That’s five moves after the first meeting at the Church of the Plague, and every single one of them requires resting at a Site of Grace — or reloading the area — before Millicent actually appears at the next stop.

The Haligtree Fork: Gold Sign vs. Red Sign at the Drainage Channel

Reach the Drainage Channel Site of Grace in Elphael and head toward the nearby pool of Scarlet Rot — the Putrid Tree Spirit guarding it deals Scarlet Rot damage with every attack, not the holy damage of its near-identical cousin the Ulcerated Tree Spirit found elsewhere in the game.[4] Kill it and two summon signs appear where it stood: gold and red.

Take the gold sign and you’re summoned into Millicent’s world to fight alongside her against her four sisters — Mary (Halo Scythe), Maureen (Treespear), Amy (twin Flowing Curved Swords), and Polyanna (Blade of Calling), all wearing the Traveler’s Set and fighting with movesets that echo their mother Malenia.[3] It plays like a chaotic four-on-two skirmish; splitting their aggro around the room’s pillars keeps you from getting collapsed on by all four at once. Win, and Millicent survives long enough to hand you the Rotten Winged Sword Insignia and drop the Unalloyed Gold Needle she no longer needs.[1][2]

Take the red sign instead and you invade her world as the antagonist. Kill her yourself and you get Millicent’s Prosthesis immediately, no fight against her sisters required.[1][2] It’s the faster, simpler option — and that’s exactly why it catches people out.

What Each Ending Actually Locks

One reward isn’t part of the fork at all: the Prosthesis-Wearer Heirloom is handed over earlier in the questline, at the Church of the Plague, regardless of which sign you eventually pick at the Drainage Channel — don’t confuse it with either end-game talisman below.[1] Every choice at the fork itself, though, removes something for that playthrough. None of them can be undone without a fresh character or a full NG+ cycle.

That Smithing Stone matters more than it looks. Using the Unalloyed Gold Needle at Malenia’s arena after helping Millicent is the only way to obtain it, and it’s a separate acquisition path from the ones in our Ancient Dragon Smithing Stone locations guide. The two talismans aren’t a wash mechanically, either. The Rotten Winged Sword Insignia’s successive-attack bonus climbs 6% then 8% then 13% and is incompatible with the normal Winged Sword Insignia, so it’s a straight upgrade only if you weren’t already running that talisman.[7] Millicent’s Prosthesis climbs a flatter 4% then 6% then 11%, decays after 1.5 seconds without attacking, but adds a flat +5 Dexterity and stacks with either Winged Sword Insignia variant — meaning a Dex-scaling build can legally stack Millicent’s Prosthesis with a normal Winged Sword Insignia for both the attack bonus and the stat boost at once, something the Rotten version can’t do.[8]

Two Things Every Other Guide Gets Wrong

First: the boss guarding the Prayer Room approach is the Putrid Tree Spirit, not the Ulcerated Tree Spirit. The two share a name pattern and a similar moveset, but they deal different damage types, and it’s an easy mix-up to repeat.[4] If a guide tells you to bring holy resistance for this specific fight, it’s describing the wrong boss.

Second, and more consequential: one of the most-cited wiki pages for this exact fork lists the reward mapping backwards from its own parent Millicent page and from an independently written walkthrough.[1][2][3] Both of those two agree the gold sign (helping) gives the Rotten Winged Sword Insignia and the red sign (challenging) gives Millicent’s Prosthesis; only the sub-page reverses it. Trust the parent page and the independent source over the sub-page, and glance at your inventory before you commit if you’re not sure which sign you’re standing on.

Can I get both the Rotten Winged Sword Insignia and Millicent’s Prosthesis on one character?
No — not through this questline. Each ending forecloses the other reward for that save, so if you want both talismans you need a second playthrough or a fresh character slot, not a different summon sign choice.[1][2]

I killed Millicent by accident at Windmill Heights — is the questline really over?
Yes, for that item chain specifically. She drops Millicent’s Prosthesis and 6,000 Runes on death, so you don’t walk away empty-handed, but the Rotten Winged Sword Insignia, the Unalloyed Gold Needle, and the Miquella’s Needle path are gone for that character.[1]

Does helping Millicent against her sisters actually save her?
